Effective interviewing bypasses the patient's subjective mood interpretation and targets objective, observable behavioral shifts:

**The contrast probe. **Instead of asking if the patient felt "abnormally good," anchor the question against their depressive baseline and focus on physiology: "Has there been a time of several days when your mood felt great and you found that you slept only two or three hours a night, but still woke up feeling completely energized?" This targets undeniable biological changes (sleep, energy, appetite) rather than subjective mood interpretations.

**The "other people" probe. **Because patients normalize their own behavior, externalizing the assessment through others' reactions is highly sensitive: "When you felt better, did people say, 'You look like you're feeling well,' or did they say, 'You're acting sort of different, strange, or too intense?'" This asks the patient to report on objective external feedback rather than internal mood states.

**Lead with behavior, not mood. **Some diagnostic frameworks recommend initiating the assessment with questions about behavioral activation and sudden increases in goal-directed activity, rather than beginning with mood elevation. For Bipolar II detection specifically, this approach may be more productive because the mood elevation component of hypomania is often subtle, while the behavioral changes are more observable (Akiskal & Benazzi, 2005, Journal of Clinical Psychiatry).

4.2 The Key Symptom Probes

Insomnia is characterized by a strong desire to sleep combined with inability to initiate or maintain sleep, resulting in next-day fatigue, cognitive exhaustion, and distress. Insomnia is common in unipolar depression (affecting 40% to 80% of individuals with MDE) and anxiety disorders.

Decreased need for sleep is a condition in which the individual achieves drastically reduced sleep (often only 2 to 4 hours) but awakens feeling entirely refreshed, highly energized, and ready for vigorous activity without evidence of daytime fatigue.

This distinction reflects a disruption in the brain's sleep-wake balance specific to bipolar disorder: normally, the longer a person stays awake, the stronger their biological drive to sleep becomes (driven by the buildup of a brain chemical called adenosine). During hypomania and mania, this sleep drive appears to be significantly weakened, allowing the brain to sustain high energy output despite very little sleep.

**The two-part probe: **First, establish the quantitative change: "During that period, how many hours of sleep were you getting per night?" Second, establish the qualitative impact: "How did you feel the next day? Were you exhausted and dragging, or did you feel completely rested and ready to go despite only getting three hours of sleep?".

Increased Goal-Directed Activity

Distinct increases in goal-directed activity, whether social, occupational, or sexual, are a hallmark of elevated mood states. Patients may initiate multiple ambitious projects simultaneously, write extensively, engage in obsessive organizing, or suddenly increase their workload to unsustainable levels.

The clinician should differentiate this from the purposeless psychomotor agitation (restless, driven physical movement without productive output) of anxious depression, which involves hand-wringing, pacing, or inability to sit still driven by internal tension. Hypomanic activity, by contrast, is constructive, albeit often chaotic and unfinished.

**Effective probe: **"During that period, did you start any new projects? Were you much more active than usual, taking on lots of different tasks at the same time and feeling like you could work all day and night without needing a break?"

Pressured Speech, Grandiosity, and High-Risk Behaviors

**Pressured speech and flight of ideas: **Patients may lack insight into their own speech patterns, so questions should focus on the subjective experience of cognitive speed and others' reactions: "Did you have the sense that it was hard to stop talking once you started?" and "Did people tell you that you were talking faster than normal or that it was difficult to get a word in?"

**Grandiosity and high-risk behaviors: **Evaluating impulsive behaviors requires a non-judgmental, normalizing approach to mitigate the shame that often suppresses reporting. Patients are frequently deeply embarrassed by financial devastation or relationship damage caused during episodes:

• Spending: "Did you engage in sudden buying sprees or make investments that were completely out of character for you?"

• Sexual behavior: "During this time, did you find yourself doing things sexually that you wouldn't normally do, or acting in ways that you might usually find embarrassing?"

• Grandiosity: "During that period, did you feel you were much more capable and confident than is normal for you, far above your usual baseline?"

**A note on concordance: **Studies investigating patient-clinician agreement on MDQ items have found poor concordance for "irritability" (kappa = 0.12, where kappa measures agreement between two raters on a scale from 0 = no agreement to 1 = perfect agreement) and "racing thoughts" (kappa = 0.15), primarily because patients misinterpret the clinical definitions of these terms (Goldberg, Garakani, & Ackerman, 2012, Journal of Clinical Psychiatry). This underscores the importance of behavioral probes over diagnostic vocabulary.

4.3 The Collateral History Imperative

Given the ego-syntonic nature of hypomania, the subjective normalization of symptoms, and the state-dependent memory deficits documented in Section 1.4, systematic collateral history from family members, spouses, or close contacts is not a helpful adjunct; it is strongly recommended across all major guidelines. In practice, collateral informants are not always available at the time of evaluation: patients may present alone, family may be geographically distant, or the patient may not consent to involving others. When collateral history cannot be obtained, the evaluation should proceed, but the clinician should document the increased diagnostic uncertainty and make active efforts to obtain informant data at subsequent visits.

**Evidence of impact. **The McLean Collateral Information and Clinical Actionability Scale (M-CICAS) pilot study systematically quantified the value of collateral data (Owoyemi, Salcone, King, et al., 2021, JMIR Mental Health). When collateral information was obtained:

• Clinicians gained new insight into the patient's current clinical status in 78% of visits

• Clinicians learned new information about baseline functioning in 38% of visits

• Crucial new knowledge about past clinical history emerged in 33% of visits

• Clinicians actively adjusted medication or treatment plans as a direct result of collateral data in 34% of visits

Studies evaluating collateral informant reliability in complex populations (such as those with comorbid substance dependence and bipolar disorder) have found high levels of agreement between collateral reports and objective biological measures (urine screens), demonstrating the reliability of external informants when patient insight fails.

**What collateral informants excel at: **Identifying the objective behavioral manifestations of hypomania that patients minimize or normalize. A spouse is acutely aware that the patient stopped sleeping for five days, spent thousands of dollars on impulsive purchases, and became intensely irritable when questioned, even if the patient recalls that period as "finally feeling energized."

**What collateral informants may miss: **Internal emotional suffering, the nuanced dynamics of family functioning, and the subjective distress of depression. Collateral history is most valuable for capturing the objective criteria of mania and hypomania.

**Family history as a diagnostic anchor. **Beyond reporting on the patient's symptoms, family members are essential for establishing multigenerational psychiatric history. A meta-analysis of the family history method found that when a relative reports a family member has mania or bipolar disorder, the validity of that report (confirmed by direct interview) is very high (OR = 64), and similarly high for schizophrenia (OR = 148) (Hardt & Franke, 2007, European Psychiatry). In other words, family members are highly reliable informants for these specific conditions. A positive family history of bipolar disorder is one of the strongest indicators raising the pre-test probability of a bipolar diagnosis in a depressed patient.

4.4 Systematically Assessing Mixed Features

The DSM-5-TR replaced the old "mixed episode" diagnosis (which required simultaneously meeting full criteria for both mania and depression) with the more clinically useful "with mixed features" specifier. This specifier can be applied to manic, hypomanic, or major depressive episodes when at least three concurrent symptoms of the opposite pole are present for the majority of days during the episode.

**Prevalence and clinical significance. **Approximately 30% to 40% of patients with bipolar disorder experience mood episodes with mixed features during the course of their illness (CANMAT/ISBD 2018, Yatham et al., Bipolar Disorders; Vieta et al., 2018, Lancet Psychiatry). Mixed episodes are consistently associated with poorer clinical outcomes, greater treatment resistance, higher comorbidity burden, and critically elevated suicide risk. The combination of severe depressive hopelessness with the impulsive energy of mania creates a high-risk clinical state.

**The DSM-5-TR exclusion controversy. **The current criteria exclude overlapping symptoms (psychomotor agitation, irritability, and distractibility) from counting toward the mixed features specifier because these symptoms can occur in both pure depression and pure mania. If three or more concurrent opposite-pole symptoms are present, the clinical picture changes fundamentally: antidepressant monotherapy becomes dangerous, and atypical antipsychotics or mood stabilizers become the appropriate treatment targets. Mixed features also significantly elevate suicide risk, requiring heightened monitoring.

4.5 Measurement-Based Care: Mood Charting as a Diagnostic and

Monitoring Tool

Because retrospective recall is unreliable and bipolar disorder requires longitudinal assessment, measurement-based care using prospective mood charting is increasingly recognized as a clinical standard.

The NIMH Life Chart Method (LCM) is the traditional gold standard. Patients chart their mood severity daily, anchored to functional impairment, alongside medication adherence, sleep duration, and life events. Validation studies show strong correlations with the Inventory of Depressive Symptomatology-Clinician Rated (IDS-C) (r = -0.785) and the Young Mania Rating Scale (r = 0.656) (Denicoff et al., 2000, Psychological Medicine). However, the paper-based LCM suffers from poor adherence in clinical practice due to its resource-intensive demands.

Digital mood monitoring has emerged to address adherence barriers. The clinical interpretation: digital mood tracking is an excellent assessment and monitoring tool but not a standalone therapeutic intervention. Its value lies in empowering patient self-awareness, identifying individualized prodromal triggers, and providing clinicians with objective longitudinal data to inform pharmacological decisions. Preliminary evidence suggests that mood tracking apps may achieve larger effect sizes when paired with human guidance (clinicians actively reviewing the data) or structured CBT modules, though the evidence base for these combined approaches is still developing and specific effect sizes vary across studies.

**A practical caution: **Daily mood monitoring is not psychologically benign. A separate systematic review by the same research group (Astill Wright, Monk-Cunliffe, Guo, & Morriss, 2026, JMIR Mental Health), examining 77 studies with over 17,000 participants, noted a 4% prevalence of adverse events, including increased psychological burden from daily illness reminders, paradoxical mood worsening from hyper-fixation on symptoms, and rarely, self-harm. Clinicians should personalize the recommendation and monitor its psychological impact.

In plain language for families: Mood charting is like keeping a

weather log for emotions. Just as meteorologists need weeks and months of data to spot patterns that no single day's forecast can reveal, clinicians need a running record of mood, sleep, and energy to understand how bipolar disorder behaves in each individual patient. The chart itself does not treat the illness, but it gives the treatment team the data they need to adjust medications accurately and catch early warning signs of an episode before it fully develops. Phone apps make this easier than the old paper charts, but the key is consistency: even brief daily entries are more useful than occasional detailed reports.

4.6 Suicide Risk Assessment During Bipolar Evaluation

Bipolar disorder carries the highest suicide risk of any psychiatric diagnosis. Lifetime suicide attempt rates among patients with bipolar disorder are estimated at 25% to 50%, and the suicide rate is approximately 15 to 20 times the general population rate (CANMAT/ISBD 2018, Yatham et al., Bipolar Disorders). Every bipolar evaluation must include a suicide risk assessment, and that assessment must account for the mood-state-specific nature of bipolar suicide risk.

**Risk varies by mood state. **Suicide risk in bipolar disorder is not constant. The highest-risk periods are depressive episodes and mixed episodes, not manic episodes. Mixed episodes are particularly dangerous because the combination of depressive hopelessness with the impulsive energy, agitation, and reduced inhibition of mania creates the capacity and the drive to act on suicidal thoughts simultaneously. Pure manic episodes carry lower suicide risk but elevated risk of accidental death due to impulsivity and poor judgment (CANMAT/ISBD 2018, Yatham et al., Bipolar Disorders).

Safety planning for bipolar patients should be mood-state-specific: the warning signs and coping strategies during a depressive episode may be different from those during a mixed or manic episode.

**A treatment selection implication. **Meta-analytic evidence consistently supports a significant anti-suicidal effect of lithium in bipolar disorder (Cipriani et al., 2013, BMJ). Whether this effect operates through a mechanism independent of mood stabilization or is primarily mediated by mood stability itself remains debated. Either way, the clinical data suggests that lithium should be factored into treatment selection for bipolar patients with a history of suicidal ideation or attempts. Warning: The Inpatient-to-Outpatient Transition

The period immediately following discharge from a psychiatric hospitalization for a mood episode is one of the highest-risk windows for suicide in bipolar disorder. Clinicians managing this transition must ensure close follow-up (ideally within 72 hours of discharge), a clear safety plan, confirmation that outpatient appointments are scheduled, and communication between inpatient and outpatient teams. This transition period warrants heightened monitoring regardless of how stable the patient appears at discharge.
