The old approach of "get sober first, then treat the mood disorder" does not work and actually makes both conditions worse. The goal of mood stabilization is not just to feel better emotionally; when the mood swings are controlled, the drive to self-medicate often decreases on its own.

The treatment approach follows a clear priority: stabilize the mood first, because trying to treat anxiety without a stable mood foundation tends not to work well and can actually make things worse. Therapy, specifically cognitive-behavioral therapy, is one of the safest and most effective tools available because it works on anxiety without any risk of triggering a mood episode. If anxiety medications are needed, the choice of which one and when to introduce it requires careful consideration by a provider who understands the bipolar context.

ADHD and bipolar disorder can genuinely occur together, and when they do, both need treatment. But the order matters enormously. Think of it like building a house: you have to pour the foundation (mood stabilization) before you can frame the walls (ADHD treatment). If you try to treat the ADHD first with stimulant medications before the mood is stable, the stimulant can push the mood in a dangerous direction. Once the mood is solidly stabilized, stimulants can usually be added safely with close monitoring. The key warning sign to watch for is a sudden change in sleep: if your child or family member starts sleeping much less but seems full of energy rather than tired, that is a signal to contact the prescriber immediately, as it may indicate the medication is pushing toward a manic episode.

The days and weeks right after someone leaves the hospital are actually the most dangerous period, not the safest. The hospital provided constant structure and monitoring. Coming home means returning to the stresses that may have contributed to the crisis, often while adjusting to new medications. The most important thing a family can do during this transition is ensure the very first outpatient appointment happens within a few days of discharge, not weeks. If the person was started on lithium, making sure they continue taking it exactly as prescribed is critical, because stopping lithium suddenly can trigger a dangerous rebound in suicidal risk. During this transition period, removing firearms from the home, locking up any stockpiled or non-prescribed medications and potentially dangerous household items (while keeping the person's own prescribed psychiatric medications accessible for daily use, with a trusted family member monitoring adherence), and maintaining close daily contact are concrete, evidence-based actions that save lives.

6.3 Postpartum Psychosis: A Psychiatric Emergency

Women with bipolar disorder have the highest risk of any psychiatric population for developing postpartum psychosis, estimated at 20% to 25% (a more than 100-fold increase over the general population rate of 1-2 per 1,000 deliveries) * (Wesseloo et al., 2016, American Journal of Psychiatry)*.

Clinical presentation. Postpartum psychosis is entirely distinct from postpartum depression. The onset is rapid and fulminant, typically erupting within the first 3 to 14 days after delivery. The presentation is frequently described as "delirium-like":

• Waxing and waning confusion, disorientation, and cognitive disturbance

• Severe mood lability oscillating between euphoria, severe dysphoria, and agitation

• Severe insomnia, disorganized behavior, and paranoid ideation

• Ego-syntonic delusions and hallucinations, often focused on the newborn (e.g., beliefs that the infant is demonic, defective, or needs to be "saved" through death)

Because the psychotic beliefs are ego-syntonic (the mother genuinely believes them and does not resist them), postpartum psychosis carries an estimated 5% risk of suicide and a 4% risk of infanticide * (Sit et al., 2006; Bergink et al., 2015, Biological Psychiatry)*. Suspected postpartum psychosis mandates immediate emergency evaluation, inpatient psychiatric hospitalization, and temporary separation of mother and infant to ensure safety.

The critical differential: postpartum psychosis vs. perinatal OCD

All non-psychiatrist providers working with new mothers must be able to distinguish the delusions of postpartum psychosis from the intrusive thoughts of perinatal OCD. Misdiagnosing OCD as psychosis leads to traumatic, unwarranted hospitalizations. Misdiagnosing psychosis as anxiety can result in fatal outcomes.

Perinatal OCD occurs in roughly 2% to 5% of postpartum women. Approximately 50% experience intrusive images of harming their infant (dropping, drowning, stabbing). The diagnostic distinction hinges on the psychological nature of the thoughts:

• Perinatal OCD: Thoughts are ego-dystonic (experienced as unwanted, alien, and deeply distressing). The mother is horrified by the thoughts and actively resists them. Reality testing is intact. She avoids the infant out of fear of acting on the thoughts, even though she would never do so. The risk of acting on the thoughts is essentially zero.

• Postpartum psychosis: Beliefs are ego-syntonic (experienced as reality). The mother may act on delusional beliefs without distress. Reality testing is absent. The risk of acting is real and immediate.

Perinatal OCD is appropriately treated with outpatient exposure and response prevention therapy and SSRIs, without requiring separation from the child.

6.4 Postpartum Prophylaxis: The Bergink et al. Data

Given the astronomical postpartum risk, prophylactic pharmacological intervention is a clinical necessity, not merely an option.

The premier evidence comes from Bergink and colleagues (2012, American Journal of Psychiatry), who prospectively evaluated 70 high-risk pregnant women at a single center (Erasmus Medical Center, Netherlands): 41 with chronic bipolar disorder and 29 whose prior psychotic episodes were limited exclusively to the postpartum period. The study was not randomized; women chose whether to accept prophylaxis. The small subgroup sizes (N=20 receiving prophylaxis, N=9 declining) limit the precision of the percentage estimates, though the direction of effect is clinically compelling.

Bipolar disorder cohort: Despite maintenance medication during pregnancy, 24.4% still relapsed during pregnancy. Among bipolar women who experienced a mood episode during pregnancy, the postpartum relapse rate soared to 60.0% even with prophylaxis. These women require continuous pharmacological prophylaxis throughout the entirety of pregnancy and the postpartum period.

Isolated postpartum psychosis cohort: All 29 women remained stable throughout pregnancy without medication. Immediately after delivery, 20 initiated lithium or antipsychotic prophylaxis, and zero (0%) relapsed. Among 9 who declined prophylaxis, 44.4% suffered severe postpartum relapse requiring hospitalization.

Meta-analyses synthesizing this and related data indicate that prophylactic lithium yields an NNT of 3 for preventing postpartum psychosis * (Fornaro et al., 2020)*.

Practical implementation: High-risk women must have a pre-planned, collaborative perinatal order for immediate postpartum lithium prophylaxis, frequently administered on the evening of delivery. The Bergink protocol targeted plasma levels in the therapeutic range (standard maintenance levels of 0.6-0.8 mmol/L are typically appropriate, though some clinicians target the higher end of this range given the acute risk window; this reflects clinical convention rather than a specific RCT-derived target). Sleep preservation is a non-negotiable companion intervention: using partner-assisted night feeds or nursing support to guarantee uninterrupted maternal sleep blocks, with adjunctive benzodiazepine (e.g., lorazepam 1 mg at bedtime) if needed. Severe sleep disruption is a primary neurobiological trigger for postpartum mania and psychosis.

6.5 Breastfeeding Considerations

All psychotropic medications are excreted into breast milk to varying degrees. The decision to breastfeed requires individualized risk-benefit analysis.

Lithium: Historically considered an absolute contraindication to breastfeeding. Infant serum levels can reach 30% to 50% of maternal levels. However, 2024-2025 guideline updates are increasingly permissive. The AAP and LactMed now suggest lithium can be used in breastfeeding healthy, full-term infants with strict pediatric monitoring: regular infant blood draws for serum lithium, TSH, and renal function. Clinicians must watch for neonatal toxicity signs (hypotonia, cyanosis, lethargy), particularly if the infant becomes ill or dehydrated.

Lamotrigine: Infant serum levels vary between 20% and 50% of maternal levels. Generally considered safe and compatible with breastfeeding. Monitor infants for rare apnea, poor sucking, excessive drowsiness, or rash.

Quetiapine and olanzapine: Highly favorable lactation profiles. Both are heavily protein-bound and lipophilic, resulting in extremely low transfer into breast milk. Exclusively breastfed infants ingest less than 1% of the maternal weight-adjusted dose, and infant serum levels are routinely undetectable. These are considered first-line choices during breastfeeding.

Valproate: Enters breast milk but generally produces low infant serum levels. However, carries rare risks of neonatal hepatotoxicity. If breastfeeding is pursued on valproate, infant liver function monitoring is recommended.

If you have bipolar disorder and are pregnant or planning pregnancy, the most important thing to know is that stopping your medication is usually more dangerous than continuing it. The old advice to "stop everything while pregnant" has been replaced by evidence showing that untreated bipolar disorder during pregnancy causes serious harm to both mother and baby. Most medications can be continued with appropriate monitoring and dose adjustments. The one clear exception is valproate (Depakote), which carries unacceptably high risks for the baby's brain development and should be switched to a safer medication before conception whenever possible. After delivery is the highest-risk time; having a plan in place before the baby arrives, including medication that starts the same day as delivery and a strategy for protecting your sleep, can prevent the severe psychiatric emergencies that otherwise affect roughly 1 in 4 to 5 bipolar women.

The newer bipolar depression medications (cariprazine and lumateperone) work differently from older options and tend to cause less weight gain and fewer blood sugar problems. Cariprazine works best at a low dose for depression, which is the opposite of what you might expect. Its main side effect is a feeling of restlessness that can be mistaken for anxiety. Lumateperone is one of the best-tolerated options currently available and is notable because it is approved for both Bipolar I and Bipolar II depression. Your prescriber can help determine which medication makes the most sense based on your family member's specific situation, including whether other options have already been tried and how much weight and metabolic effects are a concern.

The diagnostic bias section of this module addresses something that affects real patients: research shows that Black patients are systematically more likely to be misdiagnosed with schizophrenia when they actually have bipolar disorder. This matters because the two conditions require very different medications, and getting the wrong diagnosis means getting the wrong treatment, sometimes for years.
