Therapy, specifically cognitive-behavioral therapy, is one of the safest and most effective tools available because it works on anxiety without any risk of triggering a mood episode. If anxiety medications are needed, the choice of which one and when to introduce it requires careful consideration by a provider who understands the bipolar context.

ADHD and bipolar disorder can genuinely occur together, and when they do, both need treatment. But the order matters enormously. Think of it like building a house: you have to pour the foundation (mood stabilization) before you can frame the walls (ADHD treatment). If you try to treat the ADHD first with stimulant medications before the mood is stable, the stimulant can push the mood in a dangerous direction. Once the mood is solidly stabilized, stimulants can usually be added safely with close monitoring. The key warning sign to watch for is a sudden change in sleep: if your child or family member starts sleeping much less but seems full of energy rather than tired, that is a signal to contact the prescriber immediately, as it may indicate the medication is pushing toward a manic episode.

5.5 Mood-State-Specific Safety Planning

Because bipolar disorder involves radical physiological shifts in cognition, energy, and insight, a safety plan effective during depression may be entirely ineffective during a mixed state. Clinicians should collaborate with patients to create mood-state-specific plans.

No published protocol has yet validated a bipolar-specific, mood-state-adapted safety plan in randomized trials. The following adaptations represent an application of established safety planning principles (Stanley and Brown) to the specific phenomenology of bipolar mood states, grounded in clinical reasoning and expert consensus rather than direct RCT evidence.

Safety planning for bipolar depression

The phenomenology: severe lethargy, hypersomnia (excessive sleeping, often 12 or more hours without feeling rested), severe hopelessness, psychomotor retardation, cognitive rigidity.

Warning signs to identify: sleeping more than 12-14 hours, inability to perform basic hygiene, cessation of communication with support networks, withdrawal from previously engaged activities.

Coping strategy modifications: Due to severe anergia, strategies should be extremely low-barrier. Complex cognitive exercises will likely fail. Simple behavioral activation targets are prioritized: "I will sit by the window for 5 minutes," "I will listen to a specific comforting playlist." The goal is any micro-engagement with the environment.

Means restriction: Because the patient lacks physical energy to secure their own environment, lethal means restriction (locking firearms, removing stockpiled medications) must be explicitly managed by a designated third party.

Safety planning for mixed states (the highest-risk period)

The phenomenology: the patient experiences the negative cognitive triad of depression (hopelessness, worthlessness) while possessing the physical energy, psychomotor agitation, panic-level anxiety, and racing thoughts of mania. They feel as though they are "jumping out of their skin" while simultaneously believing life is meaningless.

Warning signs: extreme, uncharacteristic irritability, severe insomnia combined with physical exhaustion ("wired but tired"), pacing, physical restlessness, sudden surges of panic-level anxiety.

Coping strategy modifications: Standard distraction techniques (watching a movie, calling a friend) fail against severe psychomotor agitation. The plan must incorporate intense distress tolerance skills. Techniques from Dialectical Behavior Therapy (DBT), particularly the TIPP skills (Temperature change, specifically cold water applied to the face to activate the mammalian dive reflex and slow heart rate; Intense cardiovascular exercise; Paced breathing; Paired muscle relaxation), are designed to rapidly downregulate the overactive autonomic nervous system through physiological rather than cognitive mechanisms.

Escalation threshold: Because impulsivity is at its peak, the threshold for contacting professionals or proceeding to emergency services must be drastically lowered. The plan should bypass "social distraction" steps if agitation is severe.

Safety planning for mania/hypomania

The phenomenology: elevated or expansive mood, grandiosity, decreased sleep need, increased goal-directed activity, and severely impaired judgment with absent insight. The patient often feels better than ever and does not perceive themselves as being at risk.

The insight deficit: Because the patient will likely lose the capacity to voluntarily initiate their own safety plan, the plan must be highly collaborative with collateral supports. Family members and caregivers must be explicitly empowered to intervene, contact providers, or initiate hospitalization when they observe warning signs, as the patient will likely deny their presence.

Proactive harm reduction: The manic safety plan focuses on preventing severe psychosocial damage (financial ruin, relationship destruction) that could later precipitate a suicidal depression. Proactive agreements made during euthymia should include relinquishing credit cards, locking online banking access, and handing over vehicle keys to a trusted proxy when early warning signs emerge.

5.6 The Vulnerable Transition: Inpatient to Outpatient Care

The period immediately following discharge from psychiatric hospitalization is the single highest-risk interval for suicide in the bipolar illness trajectory. Suicide death rates are up to 300 times higher in the first week post-discharge and 200 times higher in the first month compared to the general population.

Nationwide cohort studies encompassing over 1.8 million patients * (Olfson et al., 2016)* have confirmed that a discharge diagnosis of bipolar or major depression carries the highest short-term post-discharge suicide risk compared to other diagnostic groups. The reasons are multifaceted: abrupt removal of 24/7 structured safety, return to precipitating psychosocial stressors, destabilizing effects of a new medication regimen, and the stigma of recent hospitalization.

Evidence-based transition practices

Partial hospitalization programs (PHP) or intensive outpatient programs (IOP), which provide structured daily or multi-weekly clinical contact, should be used as step-down care when standard outpatient follow-up cannot be secured rapidly or when the patient's clinical stability is marginal. These programs maintain a therapeutic structure that bridges the gap between 24/7 inpatient monitoring and the relative independence of weekly outpatient visits.

The "warm handoff." Passive referrals (giving the patient a phone number) are insufficient. A staffing conference call between the inpatient team and the outpatient clinician should occur before discharge, covering treatment course, safety plan details, and medication regimen. Institutional structures such as formal Memorandums of Understanding (MOUs) between inpatient and outpatient facilities can ensure the rapid, electronic delivery of discharge summaries, medication lists, and safety plans to the receiving provider before the patient's first visit, rather than relying on the patient to carry this information.

Active contact strategies. Peer support specialists (individuals with lived experience of bipolar disorder or psychiatric hospitalization), outreach phone calls during the gap days between discharge and the first appointment, and practical assistance with the first outpatient visit (such as meeting the patient in the waiting room) significantly increase appointment adherence and safety. These strategies address the reality that a recently discharged patient in early recovery may lack the executive functioning or motivation to navigate the outpatient system independently.

Medication verification. At the first outpatient visit, verify that the patient filled their prescriptions, assess for intolerable side effects (the primary driver of abrupt discontinuation), and explicitly address lithium adherence given the severe rebound risk upon stopping.

When specialist access is limited. Many of the above practices assume infrastructure that rural and underserved settings lack: PHP/IOP programs may not exist locally, peer support specialists may be unavailable, and the first outpatient psychiatry appointment may realistically be weeks away, not days. In these settings, the primary care provider may be the de facto post-discharge clinician. Practical adaptations include: scheduling a PCP visit within 48 to 72 hours of discharge even if a psychiatry appointment cannot be arranged that quickly; using telehealth platforms for psychiatric follow-up when no local specialist is available; designating a specific staff member (nurse, medical assistant, care coordinator) to make outreach calls on days 1, 3, and 7 post-discharge; and establishing a standing protocol with the nearest emergency department for same-day evaluation if warning signs emerge. The principles (rapid contact, medication verification, active outreach) remain the same even when the specialist resources are not locally available.

Coming home means returning to the stresses that may have contributed to the crisis, often while adjusting to new medications. The most important thing a family can do during this transition is ensure the very first outpatient appointment happens within a few days of discharge, not weeks. If the person was started on lithium, making sure they continue taking it exactly as prescribed is critical, because stopping lithium suddenly can trigger a dangerous rebound in suicidal risk. During this transition period, removing firearms from the home, locking up any stockpiled or non-prescribed medications and potentially dangerous household items (while keeping the person's own prescribed psychiatric medications accessible for daily use, with a trusted family member monitoring adherence), and maintaining close daily contact are concrete, evidence-based actions that save lives.

If you have bipolar disorder and are pregnant or planning pregnancy, the most important thing to know is that stopping your medication is usually more dangerous than continuing it. The old advice to "stop everything while pregnant" has been replaced by evidence showing that untreated bipolar disorder during pregnancy causes serious harm to both mother and baby. Most medications can be continued with appropriate monitoring and dose adjustments. The one clear exception is valproate (Depakote), which carries unacceptably high risks for the baby's brain development and should be switched to a safer medication before conception whenever possible. After delivery is the highest-risk time; having a plan in place before the baby arrives, including medication that starts the same day as delivery and a strategy for protecting your sleep, can prevent the severe psychiatric emergencies that otherwise affect roughly 1 in 4 to 5 bipolar women.

Lumateperone is one of the best-tolerated options currently available and is notable because it is approved for both Bipolar I and Bipolar II depression. Your prescriber can help determine which medication makes the most sense based on your family member's specific situation, including whether other options have already been tried and how much weight and metabolic effects are a concern.

The diagnostic bias section of this module addresses something that affects real patients: research shows that Black patients are systematically more likely to be misdiagnosed with schizophrenia when they actually have bipolar disorder. This matters because the two conditions require very different medications, and getting the wrong diagnosis means getting the wrong treatment, sometimes for years. If you or a family member is Black and has received a schizophrenia diagnosis, but the psychotic symptoms seem to come and go with mood changes rather than being constant, it may be worth asking the treatment team specifically about whether bipolar disorder with psychotic features should be considered.
