This final section addresses the controversies that generate the most active disagreement among experts: the role of antidepressants in bipolar depression, the question of whether maintenance treatment should be lifelong, and how international guidelines diverge on fundamental treatment decisions.

9.1 The Antidepressant Debate

The use of antidepressants in bipolar depression remains one of the most contentious topics in psychiatric practice. The controversy is older than most trainees: the landmark STEP-BD trial (Sachs et al., 2007, New England Journal of Medicine) randomized bipolar patients already receiving mood stabilizers to adjunctive antidepressant or placebo and found no significant advantage for antidepressant addition in achieving depression remission (Sachs et al., 2007, NEJM). This finding cast a restrictive shadow over antidepressant prescribing for bipolar depression that persists today. More recent data has complicated the picture without resolving it. A Danish target trial emulation (Rohde et al., 2024) (using health registry data to simulate a randomized trial design in 979 patients discharged from inpatient bipolar depression treatment) found no statistically significant association between antidepressant prescription and subsequent manic or hypomanic hospitalization (Rohde et al., 2024, American Journal of Psychiatry). 9.3 Should Maintenance Treatment Be Lifelong?

The kindling hypothesis (Post, updated through 2026) posits that untreated mood episodes progressively sensitize the brain, lowering the threshold for future episodes until they occur autonomously. This model supports early, aggressive, and indefinite maintenance treatment. But many patients, particularly during extended euthymic periods, question whether lifelong medication is truly necessary. In bipolar I, adjunctive antidepressant use carries substantially higher risk and should be reserved for refractory cases after multiple guideline-concordant options have failed. In either subtype, antidepressant monotherapy without a mood stabilizer remains contraindicated.
