Therapy, specifically cognitive-behavioral therapy, is one of the safest and most effective tools available because it works on anxiety without any risk of triggering a mood episode. If anxiety medications are needed, the choice of which one and when to introduce it requires careful consideration by a provider who understands the bipolar context.

ADHD and bipolar disorder can genuinely occur together, and when they do, both need treatment. But the order matters enormously. Think of it like building a house: you have to pour the foundation (mood stabilization) before you can frame the walls (ADHD treatment). If you try to treat the ADHD first with stimulant medications before the mood is stable, the stimulant can push the mood in a dangerous direction. Once the mood is solidly stabilized, stimulants can usually be added safely with close monitoring. The key warning sign to watch for is a sudden change in sleep: if your child or family member starts sleeping much less but seems full of energy rather than tired, that is a signal to contact the prescriber immediately, as it may indicate the medication is pushing toward a manic episode.

During this transition period, removing firearms from the home, locking up any stockpiled or non-prescribed medications and potentially dangerous household items (while keeping the person's own prescribed psychiatric medications accessible for daily use, with a trusted family member monitoring adherence), and maintaining close daily contact are concrete, evidence-based actions that save lives.

If you have bipolar disorder and are pregnant or planning pregnancy, the most important thing to know is that stopping your medication is usually more dangerous than continuing it. The old advice to "stop everything while pregnant" has been replaced by evidence showing that untreated bipolar disorder during pregnancy causes serious harm to both mother and baby. Most medications can be continued with appropriate monitoring and dose adjustments. The one clear exception is valproate (Depakote), which carries unacceptably high risks for the baby's brain development and should be switched to a safer medication before conception whenever possible. After delivery is the highest-risk time; having a plan in place before the baby arrives, including medication that starts the same day as delivery and a strategy for protecting your sleep, can prevent the severe psychiatric emergencies that otherwise affect roughly 1 in 4 to 5 bipolar women.

The following vignettes illustrate the diagnostic and treatment dilemmas covered in this module. Every clinical detail is drawn from the evidence presented in the preceding sections.

Vignette 1: The Chronically Irritable Child: Bipolar or Not?

Presentation: Marcus, age 8, is referred by his school counselor for evaluation of "bipolar disorder." He has daily explosive tantrums lasting 20-45 minutes, is chronically irritable with an angry baseline mood, and has been suspended three times for physical aggression. His mother describes him as "always angry" since age 4. He sleeps 9-10 hours nightly and wakes tired. He has comorbid ADHD diagnosed at age 6, currently on methylphenidate with partial response. There is no family history of bipolar disorder. His mother reports no periods where Marcus was "unusually happy, silly, or full of energy" in a way that was out of character.

Clinical reasoning: The critical question is whether Marcus's irritability is episodic (a departure from baseline, suggesting bipolar mania) or chronic and persistent (suggesting DMDD, ODD, or ADHD-related dysregulation). His irritability has been continuous since age 4 with no distinct episodes, no euphoria or grandiosity, no decreased sleep need, and no periods of remission (Section 1.2-1.3). His presentation maps precisely onto the broad phenotype (SMD/DMDD), which longitudinal data shows is distinct from bipolar disorder and predicts future unipolar depression rather than mania (Section 1.3: Brotman et al., OR 7.2 for future depression). At age 8, true bipolar disorder is exceedingly rare; the median onset clusters around 18.2 years for BD-I (Section 1.5).

Management approach: A bipolar diagnosis is not supported. He meets criteria for DMDD and comorbid ADHD. The evidence-based treatment sequence is: (1) optimize ADHD pharmacotherapy, as uncontrolled ADHD is a major driver of secondary irritability (Section 1.4: Zhang et al., 2024 meta-analysis); (2) initiate exposure-based CBT targeting irritability and frustration tolerance; (3) reserve atypical antipsychotics only for severe physical aggression after behavioral interventions and ADHD optimization have been tried (Section 1.4). His clinician should avoid labeling him with bipolar disorder, which would expose him to mood stabilizers and antipsychotics without evidence of benefit and with substantial metabolic risk in a growing body (Section 1.7: fourfold increased diabetes risk with SGAs in youth).

Vignette 2: The Bipolar Woman Planning Pregnancy

Presentation: Amara, age 31, has Bipolar I disorder, stable on lithium 900 mg/day (serum level 0.78 mmol/L) and quetiapine 300 mg for the past three years. She and her husband are planning their first pregnancy. She is anxious about "poisoning the baby with lithium" and asks whether she should stop all medications before conceiving. Her last mood episode was a severe mixed state requiring hospitalization four years ago. She has a family history of postpartum psychosis (her mother).

Clinical reasoning: The medication discontinuation question is the central dilemma (Section 6.1). The Viguera et al. data shows a 70-80% relapse rate with discontinuation vs. 30-35% with continuation, and abrupt discontinuation carries a 50% relapse risk within 2 weeks. Her history of severe mixed states and family history of postpartum psychosis place her at extremely high risk. Regarding lithium specifically: the Patorno et al. (2017) data recalibrates the cardiac malformation risk to an absolute 0.6-0.7% for Ebstein's anomaly at her dose (900 mg/day is in the highest risk tier with RR 3.22, Section 6.2). Dose reduction to ≤600 mg/day would bring the RR close to baseline (RR 1.11, non-significant). Quetiapine carries a moderate gestational diabetes risk (adjusted RR 1.28, Section 6.2) but is not a structural teratogen. Her family history of postpartum psychosis makes postpartum prophylaxis planning essential.

Management approach: (1) Counsel against discontinuation; the relapse risk vastly outweighs the recalibrated teratogenic risk, especially given her severe illness history. (2) Discuss whether lithium dose reduction during the first trimester might reduce teratogenic risk, weighing the Patorno dose-response data (doses ≤600 mg/day showed no statistically significant cardiac risk increase) against the competing risk of mood destabilization during a period when 47% of relapses occur. This is a collaborative clinical risk-benefit judgment, not a guideline recommendation. (3) Schedule a targeted fetal echocardiogram at 18-22 weeks (Section 6.2: ACOG recommendation). (4) Increase lithium dose monitoring to every 4 weeks during pregnancy as GFR changes alter clearance. (5) Plan for immediate postpartum lithium prophylaxis given her family history (Section 6.4: Bergink et al. data, NNT of 3 for preventing postpartum psychosis). (6) Develop a postpartum sleep preservation plan with partner-assisted night feeds. (7) Do not routinely hold lithium at delivery (Section 6.2: Molenaar et al. 2021 data), but institute twice-weekly TDM in the first two postpartum weeks. (8) Monitor for gestational diabetes given quetiapine use. (9) Quetiapine is a first-line breastfeeding choice if she wishes to nurse (less than 1% of maternal dose transferred).
